Eligibility Requirements

  • 18 years of age or older
  • Live in one of the following NRHA communities:
    • Calvert Square
    • Diggs Town
    • Oakleaf Forrest
    • Young Terrace

How It Works

  • Application & Intake: After you submit your application, a staff member will contact you for a brief screening. You’ll then meet one-on-one with a Navigator to assess your goals.

  • Care Coordination
    With your Navigator’s guidance, you’ll set personalized goals and get connected to resources that support your journey.

  • Closing the Loop: Receive weekly support to make meaningful strides toward success.
